What the carbon is actually doing
The throttle body is a valve. A flat plate sits in a bore, and how far it opens decides how much air the engine gets. At cruise that plate is well open and a little grime makes no difference. At idle it is nearly closed, and all the air the engine breathes is squeezing through a gap of a few thousandths of an inch around the edge of the plate.
Soot builds in exactly that gap. It comes from the crankcase ventilation system and the exhaust gas recirculation system, both of which route oily vapor back into the intake by design. Over tens of thousands of miles that vapor bakes into a lacquer around the bore, and the gap that governs your idle gets smaller.
The engine computer compensates, opening the plate a fraction further to keep idle speed where it belongs, and for a long while you notice nothing. Then the compensation runs out of range. That is when the symptoms arrive all at once: an idle that hunts up and down, a stall when you come off the freeway and put the clutch in, a flat spot when you ease away from a stop sign. The car did not break yesterday. It ran out of adjustment yesterday.
Cleaning the bore and the edge of the plate restores the airflow the software was written around. On an engine with no other faults, the change is immediate and obvious.

Why spraying it can leave you worse off
Almost every vehicle built in the last fifteen years has an electronic throttle. There is no cable from the pedal. A motor inside the throttle body moves the plate, and two position sensors tell the computer exactly where it is, because a disagreement between them is a safety shutdown.
That changes what a cleaning is. Aerosol carburetor cleaner aimed into the bore can strip a protective coating some manufacturers apply, and it can run down past the plate into the motor and the sensor contacts. Forcing the plate open by hand to reach behind it strains the gear train that drives it. Either one converts a small maintenance job into a replacement throttle body, which is a part with a price nobody enjoys.
The safe version is unhurried: correct cleaner on a rag rather than sprayed in, the plate opened electronically or left alone, no scraping of the bore surface, and nothing allowed to run down into the electronics. It is not complicated work. It is work that punishes shortcuts.

The step most people skip afterwards
A cleaning without a relearn is why some vehicles idle worse after the job than before it. The computer is still holding the compensation it built up for a dirty throttle, and now the throttle is clean, so the idle sits high or surges.

- Confirm the trims settledFuel trim values after a short drive say whether the idle problem is solved or whether something else, a vacuum leak or a contaminated air sensor, is still pulling on it.
The air sensor that ruins the result
A mass airflow sensor sits in the intake tract and measures incoming air with a heated element. Oil is its enemy, and the usual source is an over-oiled aftermarket performance filter: the oil migrates downstream, coats the element, and the sensor under-reports airflow forever after. Clean the throttle body on that engine and the hesitation stays, because the fault was upstream. These are the jobs that sit alongside this one.

How often does a throttle body need cleaning?
There is no fixed interval, because it depends almost entirely on how the vehicle is used. A car doing short trips in traffic, a delivery van stopping every few minutes, or a truck idling on a jobsite all collect deposits far faster than a vehicle that runs long highway stretches. In practice the symptoms are the schedule: when the idle starts hunting or stalling appears as you slow down, it is time regardless of mileage.
Can I just spray cleaner into the intake myself?
On an electronic throttle that is the move that turns a small job into a large one. Aerosol sprayed into the bore can strip a factory coating and run down into the motor and the position sensors behind the plate, and levering the plate open by hand strains the gears that drive it. A replacement throttle body costs many times what a proper cleaning does. Correct solvent on a rag, by hand, with nothing allowed to run inward, is the version that works.
Why does my idle seem worse after a cleaning?
Because the computer is still compensating for the dirt that is no longer there. It had learned to hold the plate slightly further open to keep idle correct with a restricted bore, and with the bore clean that same position now passes too much air. The adaptation has to be cleared and the make’s relearn procedure run. Done properly, the idle is right within minutes rather than after days of the vehicle sorting itself out.
What does the mass airflow sensor have to do with it?
It measures the air entering the engine, and it is easily contaminated. The most common cause is an aftermarket performance filter that has been over-oiled, which sends oil downstream onto the sensor’s heated element. Once coated, it reports less air than is really flowing and the engine runs lean, producing hesitation and a stumble that feel exactly like a dirty throttle body. Testing it is part of this job, because cleaning the throttle alone would not fix it.
Will this fix a rough idle every time?
No, and anyone promising that is guessing. Carbon at the throttle plate is a very common cause of hunting idle and stalling, but a vacuum leak, worn plugs, a failing coil, a contaminated airflow sensor or a sticking ventilation valve all produce similar complaints. Reading live data first tells us which one is in play. If the throttle body turns out to be clean, we say so and keep looking instead of cleaning it anyway.
Does this need a shop?
It does not. The intake ducting comes off, the bore and plate get cleaned by hand, everything goes back with its seals seated, and the relearn runs off a scan tool. All of that happens in a driveway, a parking lot or a yard with the vehicle exactly where it already is, which is the whole point of a mobile setup for a job this size.
Stalling at stops is worth sorting now.
